Hi, Bolivia Team!


Here is the information for your visa application, which we went through together on the phone call. I know that for most of you not all the blanks were completed. So please print this out and use this email to fill out all the blanks in the application.


Please print in COLOR and use blue or black Ink.


At the airport in Bolivia you will need to present:


-this application


-your two passport photos


-your passport


-your yellow fever card


Your leaders will have your Letters of Invitation and visa fee.


 


Here are your instructions for the visa application:


 


-Check the box for “tourist�


-In the first row, write your last name, then first name, then your middle name.


-Second row: write your birthdate and place of birth, nationality, and “check� that your marital status.


-Third row: write your passport information.


-Final box on first page: write your personal contact information


 


Second page:


-Indicate if you’ve ever applied for a Bolivian visa before. If you have, say where in the box below, to the left.


-You plan to be in Bolivia for 19 days.


-Next row of boxes: You will visit San Isidro and Santa Cruz


-The box beside that, indicate if you have family in Bolivia.


 


In the next set of boxes: complete your emergency contact information


- In the box marked “3�, circle “plane�. Routa: AA922. Date of return: July 23, 2010


-Final row of boxes: place of application is “airport/VVI�. Day: 06 Month: 07 year: 2010


Make sure you remember to sign it!


 


 


***DO NOT FORGET YOUR EXIT FEE OF $25. That is NOT included in the cost of the trip!!! Please bring it in cash to Training Camp to give it to your leaders, who will carry it while in Bolivia. This way you won’t spend it on souvenirs. :0)
